The PFK Care Coordination Team lead works closely with the Program Manager to ensure all aspects of the care coordination program including care transitions, case management and ED diversion programs meet expectations. This position requires a Registered Nurse who is able to direct the work of other Registered Nurses, Social Workers, and non-clinical staff.
