　　Are you looking for an opportunity to advance your career while working with an extraordinary team?  At Merakey, we put heart and soul into everything we do.We are seeking aThe Bilingual Integrated Health Care Navigator to join our team in our Philadelphia, PA 19122 location.Earn $47,000/year.Position DetailsThe Bilingual Integrated Health Care Navigator will be responsible for performing risk assessments, risk reduction education, and outreach with all individuals receiving integrated health services. They will also collaborates with medical providers and nursing staff to communicate test results and reports positive test results to Philadelphia Department of Public Health, pharmacy and medical staff to secure prior authorization for medications, ensure timely delivery of patient's medication, and complete successful medical service referrals. 　　Requirements

　　The ideal candidate will possess the following qualifications:High School Diploma. Bachelor's Degree is preferred.One (1) year experience as a Patient Navigator, Case Manager, Health Educator, or Care Manager.One (1) year experience working in Hepatitis C and HIV prevention and treatment, PrEP/PEP services, and treatment of other STIs; and with individuals with socially, culturally, and economically diverse experiences/backgrounds.Preferred experience working with individuals with a substance use disorder; and working with a FQHC or community mental or behavioral health environment.* Previous experience using an Electronic Medical Record (EMR).* Strong computer literacy with proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint).* Seeking someone who is extremely organized, pays attention to detail, and is able to respond promptly to requests, anticipates agency needs, and maintains efficient clinical operations.* Bilingual (Spanish/English) is required.Ability to work between two Merakey facilities.
