OhioMeansJobs|Cleveland-Cuyahoga County is sourcing for a Behavioral Health Case Manager for a Mental Health company in Cleveland.  

　　Are you looking for a social service position where you can see your impact on the lives of others every day? The Company is looking for case managers to work with adults in our Permanent Supportive Housing (PSH) program, a model that helps people with long-term homelessness, mental illness and/or substance use disorders get off the streets and start the process of recovery.

　　As a case manager, you'll provide on-site social services to residents of a PSH apartment building, as well as out in the community. You will learn valuable professional skills working in your small team, while receiving training, coaching, and supervision from experienced clinical managers. FrontLine's PSH program uses the housing first model, which invites individuals with a long history of homelessness and disabilities into permanent housing, and then provides social services on-site to help them maintain their housing.

　　You will work side-by-side in the PSH buildings with an interdisciplinary team, including psychotherapists, employment specialists, physical health prescribers and nurses, and life skills coaches. Case managers use evidence-based best practices, such as motivational interviewing, trauma informed care, and harm reduction to help program participants gain independence and work towards his/her life goals.
