The AdvancedPractice Provider is an advanced practice nurse or physician assistantprepared to deliver primary health care services to patients, familiesand groups emphasizing the promotion of health and prevention ofdisease. The scope of responsibilities includes collaborating andcoordinating with physicians and other health professionals regardingclinical practice, education, consultation, research and programdevelopment. Specialized Responsibilities: 1. Performs comprehensivehealth assessments for acute and chronic patients. 2. Assesses andmanages care for acute, minor, episodic illnesses and injuries. 3.Interviews and advises patient/families regarding health/illnessprevention. 4. Performs follow up exams. 5. Provides assessment,evaluation, and management of clients with acute and chronic illnesses.6. Collaborates with patients to determine physiologic, emotional, andenvironmental needs and priorities. 7. Provides case management servicesto assist clients and families with the arrangement for, or referpatients to needed services. 8. Manages patient health concerns throughtelephone triage and consultation as needed. 9. Provides services, whichmay include the ordering of appropriate diagnostic and laboratory tests,the prescription of pharmacological agents, and othernon-pharmacological treatment modalities. 10. Maintains and reviewspatient records, charts, and other pertinent information. 11. Performspatient callbacks, reports diagnostic findings and providesconsultation. 12. Participates with physicians in review of patient\'shealth records. 13. Participates in call coverage when required. 14.Attends required department meetings and participates in committees asrequested. 15. Other appropriate clinical duties as assigned bysupervisory physician(s) in accordance with experience. 16. Promotesongoing quality improvement activities. 17.
