A Place to Be Proud.Responsibilities* Assists patients and families with unmet social needs and improves patient and family access to community resources.* Collaborates with clinical staff, linking patients and families to essential support services including housing, nutrition assistance, utilities, transportation, and other identified needs.* Assists in conducting follow-up calls to patients and families to confirm linkage with identified community resources and services, assesses potential barriers, and aids in navigating resources and services.* Provides appropriate documentation of patient and family interactions in the electronic medical record in a timely manner.* Develops an expertise in making referrals to housing resources, food pantries, utility companies, government agencies, treatment facilities and other internal and external resources, including understanding programs' eligibility criteria, the intake process, and collects and reports data on program activities.* Complies with all AmeriCorps program guidelines by maintaining service hours, training hours, and service projects as outlined in the AmeriCorps Service Agreement.* Interacts in a positive, professional, and cooperative manner with families, and other center staff, refers appropriate questions or concerns to AmeriCorps Program Director.* Attends all required training courses and staff meetings and participates in AmeriCorps service projects.QualificationsKnowledge, Skills & AbilitiesEducation Requirement: High School Diploma or equivalent, required.Skills:* Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.* Ability to work as a part of a team, with diverse populations, and with healthcare providers in
