  DESCRIPTION OF WORK

  As an Aging Case Aide 1, you will be responsible for assisting Care Managers in providing information and referral services to senior citizens to help them better understand community resources available to meet their individual needs. You will be required to have a working knowledge of the physical, cultural, and social environment of the geographical area or program to establish a rapport with the elderly to foster utilization of services. Supervision is received from a professional superior and evaluated by direct observation and an evaluation of results. Responsibilities and duties include:Assisting consumers in obtaining resources and benefits including residence, furniture, food, clothing, financial assistance or other needs.Scheduling and conducting consumer contacts/visitsDocumenting contact and/or assistance provided to consumers in the SAMS Social Assistance Management SystemAssists with administrative dutiesProviding transportation for consumers to medical, dental and for other needed services, including Court hearingsAssisting Care Managers in identifying, contacting, and providing information to placement providers or other social service providers to obtain out of home placements and/or servicesAssisting Care Managers in providing Court notices to consumersTraveling with agency vehicle is required within Cambria County and surrounding countiesReviewing data for accuracy and entering into database and spreadsheets to maintain a record-keepingCompleting intake forms for older adults via telephone or in-person office visits for participation in servicesProviding explanations of community services when available to older adults to interested partiesDetermining if requested services are available and making arrangements for older adults to receive appropriate services and support as needed

  Work Schedule and Additional Information:Full-time employmentWork hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.Salary:In some cases, the starting salary may be non-negotiable.You will receive further communication regarding this position via email.   QUALIFICATIONSMinimum Experience and Training Requirements:One year of public contact work, which involved obtaining or providing information; orAny equivalent of experience and training.Other Requirements:You must meet the .
