PeaceHealth is seeking a Advance Care Planning Manager for a Full Time, 1.00 FTE, Day position. The general salary range for this job opening at PeaceHealth is $35.67 – $53.50. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.
Advanced Care Planning Manager is responsible for the business functions, development, implementation, and optimization of the Advance Care Planning Program. This professional will partner with caregivers across clinic, hospital and community setting and across all service lines. This position will provide analysis and consultation of Advance Care Planning activities. This professional will provide, develop forecasts for the new programs or services and support new service evaluations with market research. The Manager will be responsible for identification of opportunities for operational efficiencies and development of the program analysis. This professional will continuously improve and monitor data and share findings with medical leadership and staff. In addition, this position is responsible for initiating and overseeing department program quality improvement projects including patient satisfaction.
DETAILS OF THE POSITION:
Responsible for full scope supervision of Advanced Care Planning Coordinators and Facilitators.
Provides leadership and expertise in implementing and maintaining system-wide, advance care planning (ACP) programs for PeaceHealth.
Works in collaboration with Home & Community leadership team and Medical Directors to establish ACP goals and achieve desired outcomes. Partners with stakeholders across the system to assure performance monitoring and measurement of ACP Department initiatives and outcomes; utilizes metrics to drive modification and continuous quality improvement; analysis and critical evaluation of department growth opportunities and evaluates and conducts market research regarding the development of new programs and services.
Leads, coordinates and mentors the activities of ACP facilitators and instructors to ensure consistent and quality ACP service in compliance with regulatory requirements.
Leverages technology to support patient participation, clinician documentation, storage, and retrieval of ACP related documents through the electronic medical record (EMR). Partners with information systems to develop, monitor and maintain ACP referral and scheduling system.
Provides knowledge, expertise, and leadership, to identify problem situations or issues in system care related to ACP and professional ethical standards.
WHAT YOU BRING:
Bachelor's Degree Required. Master's Degree Preferred.
Minimum of 5 years Required: Advanced care planning, chronic disease management, care coordination, bereavement, and/or palliative care and
Required: Experience should include process and quality improvement, systems redesign, quality measurement, data analysis and reporting, data integrity, team-based improvement, improvement training, and team facilitation and
Demonstrated ability and skills to lead, facilitate, design, implement, and measure improvement projects and programs under senior leadership direction on a wide array of initiatives (Required)
Experience working with virtual teams and ability to navigating a large, complex organization with multiple business or service lines (Required)
Ability to travel within the organization’s communities (Required)
