Education & Experience: AssociateDegree related to human development, prevention, and/or gerontology,plus two years of experience working with people who are older and/oradults with disabilities and/or equivalent combination of education andexperience. Required: High School diploma plus 5 years of experienceworking with people who are age sixty and older and/or adults withdisabilities License/Certifications: Valid Wisconsin Driver\'s Licenseacceptable driving record, and have access to private transportation forwork-related duties. AED/CPR Certification Or any equivalent combinationof education, training, and experience, which provides the necessaryknowledge, skills, and abilities. Essential Duties JOB SUMMARY: The roleof the Independent Living Support Program Specialist is to receivereferrals, enroll participants into the pilot program, create a careplan of services for older adults or adults with physical, intellectual,or other developmental disabilities, follow up with the participants andcreate a transition plan when the pilot program will be ending. MAJORRESPONSIBILITY: Support the planning, development, and implementation ofIndependent Living Support Pilot Program for older adults and adultswith disabilities to retain or improve functioning and to delay orprevent the need for comprehensive long-term care services. Meet one onone with potential participants to determine eligibility for theIndependent Living Support Program Pilot. Assist participants withidentifying, finding, and securing resources to assist the person toremain independent in their home. Develop a care plan with theparticipant and follow up with participants on their on-going needs on aregular basis throughout the grant period. Maintain and report data asdetermined by the Department of Health Services and ADRC. Completerequired program and participant records, documentation, usingappropriate narrative, data entry, and/or spreadsheet format. Providetransitional support to participants following the completion of theservice plan and grant allocation, and/or during the last 3-4 months ofthe pilot project. Support the planning, development, implementation,and evaluation of program supports that impact the independence andsupport needs for those wanting to remain living in their homes withoutgoing onto Long Term Medicaid funding programs. Monitor care plans andwork on sustainability of support to delay or prevent individuals fromgoing onto long term support programs. Report on utilization to the ADRCFinance Department and I & A Coordinator monthly for approval.***** OTHER EXPERIENCE AND QUALIF CATIONS: Knowledge andunderstanding of aging, disabilities, health, education and humanservice program practices, planning and administration. Knowledge ofcaregiving relationships, stressors, and needs. Knowledge of dementia,the disease process, and program\'s ability to impact the caregiver.Knowledge of the various professional and clinical disciplines workingin the areas of aging, disabilities, health and human services.Knowledge of evidence-based prevention programs for aging adults andadults with disabilities Knowledge of applicable resources throughoutthe community. Knowledge of program planning and implementation.Knowledge of community, organizations and support centers. Knowledge ofcomputer and applications and usage, including all Microsoft officeprograms. Skillful at both written and oral communications. Skillful atuse of software and design of marketing materials. Ability tosuccessfully build community relations. Ability to plan programs andmonitor progress. Ability to establish and maintain effective workingrelationships with staff, county agencies, departments, providers andthe public. Ability to understand and maintain confidentiality of filesand information. Ability to organize and prioritize work. Ability todeal tactfully with difficult situations.
