　　Admissions RN for Eddy Skilled Facilities Flexible schedule and work hours weekdays or weekend days or evening hours available if desired Primary responsibility: - Complete RN assessment of patient in the hospital for admission to subacute rehab.- Creates care plan for subacute rehab, initiates orders, and coordinates a thorough handoff to our receiving Eddy Subacute Rehab team.-Completes audits of care plans in between referrals if “downtime” Requirements: NYS RN Licensure, 1 year experience (long term care experience preferred but not required)Pay Range:$33.10 - $45.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
