　　SUMMARY

　　Under the direction of a Pharmacy Specialist and in collaboration with the Clinical Pharmacy Manager responsible for supporting vaccination and medication therapy management (MTM) efforts within their assigned territory.

　　ESSENTIAL DUTIES AND RESPONSIBILITIES

　　Completing the entire patient immunization process with steps including but not limited to:

　　Identifying adults in need of vaccination according to state laws, FDA, CDC, and ACIP recommendations

　　Screening patients for contraindications and precautions

　　Billing immunizations

　　Verifying previous vaccination history and completing IIS documentation

　　Informing each patient/guardian of potential side effects and adverse reactions, orally and in writing

　　Administering the vaccine according to the guidelines in the vaccine package insert

　　Completing all required immunization documentation including immunization consent form sign off and necessary CDC and state reporting

　　Be prepared for management of a medical emergency related to the administration of the vaccines

　　Completing VAERS reports for immunization adverse reactions

　　Utilizing OutcomesMTM to identify and document medication therapy management interventions including but not limited to:

　　Communication with patient’s home pharmacy regarding patient outreach

　　Contacting patients via phone to complete patient interview

　　Complete OutcomesMTM required documentation of medication list and any drug related problems

　　Documentation of patient outreach and follow up with home pharmacy

　　Completion of cases in OutcomesMTM and mailing of any required documentation

　　MINIMUM QUALIFICATIONS

　　Must be 21 years of age

　　Registered Pharmacist in good standing within their state of practice with immunizer certification

　　CPR certification with AED training for adults and pediatrics

　　Flexibility and ability to travel to multiple locations

　　Proficiency in administration of vaccines and appropriate record keeping

　　Competency in completing medication therapy management interventions including required documentation and use of OutcomesMTM platform

　　Minimum availability 8-12 hours per week including occasional nights and weekends

　　EDUCATION AND EXPERIENCE

　　Bachelor’s degree (BS) or Doctoral degree (PharmD)
