  Job Summary and Qualifications

  The Medical Social Worker plans, organizes, and implements social work services to patients and families, plans for discharge and aftercare, maintains contact with referring agencies and related people, and with community resources used for referral.   What you will do in this role:

  You will be interviewing patients and family members to obtain information about home environment, family relationships, health history, and patient’s personality traits

  You will be evaluating data gathered in terms of the medical plan of treatment, available social service programs, and plan a pertinent therapy program that will provide maximum benefit for each patient

  You will be performing a variety of services such as advising on social problems, arranging for discharge or post-operative care at home or in institutions, placement of patients into foster or nursing homes, and alleviation of anxieties and fears and assist patients and families to understand, accept, and follow medical recommendations

  You will be preparing and updating records for each patient and required documentation of each case and maintain records, reports, etc. as required

  You will be maintaining department policies, procedures, objectives, quality assurance program, safety, infection, and environmental control and assisting in care and maintenance of department equipment and supplies

  What qualifications you will need:

  Masters degree in Social Work

  LCSW Certification required

  Two years clinical experience

  Experience in discharge planning
