  DESCRIPTION OF WORK

  This position assists those who are returning to the community after mental health hospitalization and/or preventing premature hospitalization. Work includes supporting consumer's efforts to reintegrate into or remain in his or her community, thus fostering his or her success in recovery and in building the confidence and self-reliance needed for full independent living in the community.

  This position supports the consumer's efforts consistent with his or her Individual Service Plan (ISP) in implementing goals, plans and objectives. You will be responsible for training consumers in home safety protocols, sharing observations with Supervisor, MDT members as appropriate, documenting and/or charting observations, behaviors, and consumers progress in a timely manner. You may have the opportunity to transport consumers, when necessary, in a county vehicle to appointments.

  This position requires close contact with the consumer in his or her home environment. Workers in this capacity will work on shifts to assure 24-hour availability in a small facility in which a limited number of seriously mentally ill (SMI) consumers reside, each in their own apartment.Work Schedule and Additional Information:Part-time employment, approximately 15 to 20 hours per week.Work hours are Tuesday 8:00 AM to 4:00 PM, Wednesday 4:00 PM to 10:00 PM, and Saturday 10:00 AM to 6:00 PM.Work schedule may include on-call shifts, as well as shifts including evenings, overnight, and weekends.You will receive further communication regarding this position via email.   QUALIFICATIONSMinimum Experience and Training Requirements:Six months of experience as a Residential Program Trainee; orOne year of experience in a Human Services Agency working with the mentally disabled, the elderly, or children and youth; orTwo years of college which includes courses appropriate to the Human Services field; orAny equivalent combination of experience and training.Other Requirements:You must meet the .
