JOB REQUIREMENTS: Description JOB LOCATION: Social Services PRIMARYFUNCTION: Responsible for providing assistance to individuals, asreferred by the WIser Choice program, to form a Recovery Support Team inorder for the client to successfully achieve their goals of the SingleCoordinated Care Plan while providing continued assistance andmonitoring of the client until these services are no longer necessaryand/or appropriate. PRIMARY DUTIES: Legend: E = Essential / NE = NonEssential / N/A = Not Applicable Facilitate all aspects of initialcontact services to include, but not limited to: locate and make initialcontact with referred client, develop a preliminary Single CoordinatedCare Plan (SCCP) with the client (as applicable, transition SCCP planmay be required prior to the client\'s release), assist the client todevelop a Recover Support Team, etc. E Facilitate all aspects ofcontinuing client support services to include, but not limited to:provide case management services (transportation, appointments,independent housing search, legal advocacy, benefits, court hearings,etc.), provide 24-hour, 7-day-a-week crisis access, refer individualswith less intensive needs to applicable services, maintain contact withclient and recovery team, submit preliminary and subsequent versions ofthe SCCP, assist the client with choosing and accessing services, etc. EFacilitate all aspects of monitoring, reporting, and compliance toinclude, but not limited to: update client information, monitor, update,and respond accordingly to the SCCP, monitor service authorizations toensure they are current, request extensions of additional serviceauthorizations, monitor and document the provision of services byagencies, complete the required outcome data collection and clientsatisfaction instruments, ensure compliance with program completionmeasurement protocols, develop and document a plan that identifiessources to pay for services, negotiate the allocation of funding amongvarious system representatives, etc. E Follow legal, organizational, andcontractual requirements, laws and policies (i.e.; County Contract). EPrepare, complete, and submit required items by due dates and requiredtimelines. E Establish and maintain good communication, collaboration,and cooperation with all stakeholders. NE Implement and/or practice theduties and responsibilities of a mandated reporter of suspected childabuse under current policies and procedures, as required. E Attend allmeetings and professional development activities, as directed. NEPerform other duties as assigned.
