Perform in-home and ensure visit time fully meets the needs of thepatient.Coordinate total patient care including interdisciplinarycommunication with other health care providers to enhance continuityof care.Guide and instruct patient and their families in prescribedtherapeutic activities that are directed toward improvingindependence and functionality.Review and update the medication profile; assess patient compliance,understanding of medication regime, contraindications, and sideeffects.Complete admission, re-certification, post hospital, and follow-upassessment procedures according to established procedures.Conduct discharge assessments and ensure appropriate dischargeteaching.\Qualifications
Must be a graduate of an approved school of clinical education.
Must be a licensed PT in the state of employment.
Must have at least one year experience as a PT.
Must have a masters degree or equivalent in physical therapy.
A bachelors degree and five years physical therapy experiencemay be considered in lieu of a masters degree.Medicare home health experience is preferred.
