Training opportunities for Evidenced Based Practices,to include DBT Join the Brown County team and you could be eligible forthe Public Service Loan Forgiveness Program Essential Duties Serves asliaison with consumers/patients, families, referral sources, communityagents, third party payors and others regarding services as needed;obtains and provides accurate and timely reports of consumer/patientstatus, progress and needs. Demonstrates familiarity with patient andother rights, assures their application in treatment provided, andassists in the resolution of complaints, disputes and grievances as mayarise. Conducts psychosocial assessment, DSM V diagnosis, andrecommendations on clinical assessment of need. Develops treatment plansand related discharge documentation and planning, demonstratingcollaboration with consumer/patients, their support networks, otherdepartment staff, and alternative treatment resources as needed toassure access to necessary community resources. Complies with applicablecounty, state and federal guidelines related to the provision oftreatment and maintains accurate and timely documentation according toapplicable requirements, codes, policies and standards. Identifies theneed for and assists in the planning and development of additional orrevised treatment approaches and services as necessary. Assumesresponsibility for and participates in activities which assure evidencebased practice and further professional growth and development.Demonstrates an understanding and practice of dualdiagnosis/co-occurring approaches and works with team members to addressthese issues. Attends and positively contributes to staff meetings anddepartment performance overall. Attends in-service and outside agencytraining sessions as appropriate.
