About In-House Coordinator

  GENERAL PURPOSE:

  The In-house Coordinator (IHC) actively supports the mission and goals of Finger Lakes Donor Recovery Network (FLDRN). The IHC works with health care professionals in the assigned hospital(s) to increase organ and tissue donation. The IHC will accomplish this by identifying/evaluating potential organ donors, and by offering the opportunity for donation to potential donor families with the desired result of authorization for donation. This position will be responsible to facilitate and increase engagement of healthcare professionals to promote organ and tissue donation in the facilities they serve.

  SPECIFIC RESPONSIBILITIES:

  Identification of Potential Organ Donors and Referral Response

  Rounds daily on clinical floors and works with health care team members to identify potential organ donors.

  Provides on-site response and coordinates organ referrals during scheduled work hours.

  Communicates FLDRN plans regarding referrals, including notifying attending physicians and conducting team huddles.

  May help determine medical suitability for organ donation by obtaining laboratory studies, medical record review, and obtaining med/social history.

  Communicates case updates with FLDRN team (Medical Director, AOC, Charge, OPC, FS, HD, etc).

  Referral Management (prior to authorization for donation)

  Communicates with physicians and other health care team members regarding open referrals to obtain information regarding each patient’s hospital course and next of kin’s understanding of the patient’s condition.

  In consultation with FDLRN team, consults and collaborates with medical staff to provide optimal medical management of potential donors to maximize organ suitability for transplantation.

  Supports families/legal next of kin (LNOK) of potential organ donors by helping to facilitate therapeutic interactions and interventions.

  Works with patient care teams to maintain open communications relating to end of life care and decisions of potential donors (huddle process).

  Leads the discussion with patient’s care team to determine the plan to approach the family/LNOK and offer the opportunity of organ and tissue donation.

  Serves as a donation expert and official requestor of donation, and requests authorization from family/LNOK for organ and tissue donation, or provides notification of the patient’s first-person authorization status and FLDRN’s plan to honor the individual’s decision.

  Provides hand-off to FLDRN clinical and family services teams after authorization.

  Hospital Development

  Develop strong working relationships and knowledge of the administrative and internal structures within assigned hospital(s) to help create a positive culture of donation and drive donation outcomes.

  Responsible for providing FLDRN sponsored professional education and in-service programs within assigned hospital(s) for the nursing, medical and ancillary staff.

  Collaborates with hospital leadership to develop an annual strategic donation plan to achieve mutually agreed upon goals.

  Responsible for reviewing medical records in assigned hospital(s) for appropriate referral or case follow-up.

  Performs post donor follow-up and after-action reviews, as well as provides data reports and donation outcomes to donor champions and identified hospital staff.

  Investigates and responds to reports of hospital or FLDRN process breakdowns that impact donation.

  Collaborates with internal FLDRN departments to optimize its partnership with the assigned hospital(s).

  Communication & Documentation

  Completes and documents all appropriate notes in FLDRN’s EMR (iTransplant) as required, including referral information, huddles, referral follow-up, consent/authorization for donation.

  May assist with communication between FLDRN and hospital departments related to organ recovery activity, such as medical examiner/coroners and/or pathologists as appropriate.

  Provides additional support needed to move donation process forward.

  Completes all FLDRN paperwork associated with local donor referrals, organ donors accurately and in a timely manner.

  Keeps current on existing FLDRN clinical policies and procedures

  Demonstrates a caring and courteous attitude at all times, represents FLDRN in a professional and positive manner; demonstrates regard for the dignity and respect of all patients, donor families and other agencies.

  Maintains the confidentiality of information related to patents, donor families and other agencies

  Upholds attendance and punctuality to ensure that the hospital or FLDRN are operated in an efficient and effective manner.

  Works to promote and participate in a team approach with other staff members in order to succeed with FLDRN goals.

  Additional Responsibilities

  Attends mandatory continuing staff education and required meetings.

  Maintains updated knowledge of OPTN/UNOS policy and other regulations.

  Participates in special events related to FLDRN objectives, i.e. Donate Life Month, family support groups, and any other activities identified.

  Serves as liaison with designated groups involved in organ/tissue donation in our service area.

  Assists and supports the development of abstracts and research projects.

  REQUIREMENTS:

  Bachelor’s Degree, Licensed Registered Nurse, or Allied Health Professional required

  Minimum 3 years relevant clinical experience; or hospital support/administration, marketing, sales, customer service, public relations; or an equivalent combination of education and experience required.

  Two to three years of experience in Critical Care Nursing desired or previous experience with Nursing Leadership/Management preferred.

  Previous Organ Procurement Organization experience in hospital development, clinical or family services desired.

  Must possess excellent communication, critical thinking and creative problem-solving skills, as well as, demonstrate a high level of maturity, stamina and self-motivation. .

  Pay Range

  Pay Range: $53,500 - $74,900 Annually

  The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
