The Clinical Nurse is responsible for but is not limited to: patient triage, patient assessment, and patient education and counseling. The Clinical Nurse ensures timely and productive patient flow within Primary Care and connects patients to services as needed. Responsibilities/Duties
Provide primary care services to patients of all ages, including: phone/walk-in triage, patient education, counseling, family planning activities, coordinate appointments and referrals, assist in obtaining needed services and supplies.Provide direct patient care activities including blood pressure checks and hypertension group visits, diabetic follow-up and group appointments, retinal eye exam screenings, patient-centered education and other tasks as delegated and assigned by the Primary Care Nurse Practitioners.Provides care and management for patients with a wide range of health issues.Manage prior authorization of prescriptions and treatments as well as assist with problems concerning medication refills that are not within the routine refill process established at the health center.Maintain laboratory standards and policies for designated level at facility.
ESSENTIAL DUTIES ADDENDUM:Clinical Care Management:Assume leadership role in the health center's participation in the Chronic Care Collaborative, Phase 2 including ownership of monthly reportingAttend all learning sessions, webinars, and other mandatory participatory calls as required by the Chronic Care CollaborativeDevelop a Referral Tracking System for the Primary Care DepartmentDevelop a Hospital Discharge Tracking system, making contact with local facilities to establish better communication from hospital to Primary Care ProviderReceive and analyze all reports from insurance providers or area hospitals that give us information that patients have been hospitalized or have utilized an Emergency Room . Then contact all patients in the above category within 3 business days of learning of their hospital discharge to arrange for needed care managementReceive, review, and make recommendations regarding Focused Care Management Referral Forms submitted by providers of care in the Primary Care, Behavioral Health, and Dental DepartmentsWork with other Care Management Team members to review and accept patients into the Care Management ProgramPlan and implement strategies to reduce resource consumption and achieve positive patient outcomesMonitor Care Management Program and report to team areas that need focus and re- work processing to achieve better outcomes, developing proposals to achieve needed changeMaintain a log of all patients entered into the program and key information related to each patient for data collection purposesSubmit reports as appropriate and necessary for grants, collaborative and annual reporting purposesFunction as a resource to clinic staff regarding patient assessment and plan of careAnalyze aggregate data, address deviations from program policies, and identify potentials for growth and improvement of the program based upon individual patient responses to the program and overall patient progress as a result of the program then share this information with staff and administration Required Experience
3-5 years professional nursing experience requiredAt least two years working with patients in outpatient primary care or community-based health settings preferred Required Education
Bachelor of Nursing Degree from an accredited institution Required Qualifications
Current unrestricted Registered Nurse with appropriate Pennsylvania State LicensePrevious experience working with an Electronic Health Records databaseAbility to communicate professionally in both verbal and written formsAbility to work independently and manage multiple expectationsProficient computer skills Program Summary
