  Description

  Under general supervision, provides intake and case management services. Maintains confidentiality and ethical standards as identified by NASW, HIPAA, or other relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma-informed care environment.

  Essential Duties and Responsibilities

  Maintains a caseload of clients from the URMC Respite program of up to 15 men and maintains their case files by Agency, departmental, regulator, and funder guidelines. In addition, also provides facilitation of CCFCS’s emergency shelter telehealth program. This position also provides coverage and support as needed to other programs as needed by the program manager.

  Completes relevant screenings and assessments including but not limited to psychosocial, mental health, financial status, and employment status.

  Creates, implements, and monitors comprehensive, person-centered, trauma-informed, and solution-focused plans and goals. Works with clients on harm reduction and other related goals.

  Assists in obtaining support for clients to include but not limited to home care, medical, dental, legal, substance use treatment, mental health, Department of Human Services, clothing, and other entitlement needs.

  Assist the client in understanding and meeting the expectations of service providers, courts, and employers.

  Inform clients of opportunities for social support such as recreation, faith-based, or self-help involvement. Make available tickets, transportation, and financial assistance where appropriate.

  May deliver instructions on various topics that support the client’s ability to live independently.

  Provides ongoing evaluation, supervision, and development of program activities to assure quality and effectiveness of services provided.

  Implements and maintains procedures related to program operations.

  Submits required reports and statistics promptly to the program manager as requested.

  Collaborates effectively with program partners to heighten the efficiency of joint efforts across multiple organizations.

  Communicate with appropriate program partners, agency staff, community, and criminal justice agencies to ensure ongoing and effective referrals into programs.

  Serves as Case Coordinator for clients for program care. Works with the Nurse, Peer, and other stakeholders to ensure the client's needs are being met. Coordinates and participates in regular case conferences.

  Monitors and ensures achievement of program milestones and client outcomes.

  Oversees client recordkeeping in compliance with appropriate regulatory agencies.

  Creates and implements life skills training and education opportunities for tenants.

  Collaborates with referring agencies, funders, and all related internal staff to effectively support individual client needs.

  Maintains regular contact to coordinate client benefits and address all other treatment needs.

  Reports to service providers about significant individual behavior or events that will require an immediate response.

  Acquire concrete resources to meet individual needs.

  Complete letters of request/referral forms, and other required documents of eligibility or charitable organizations and public agencies to advocate for resources that the client is entitled or eligible for.

  Makes referrals for education, employment, legal issues, and others as needed.

  Document the need for financial assistance or other resources such as wrap-around from the project to stabilize client finances.

  Utilizing various databases and/or online platforms, maintaining all documentation by the agency and appropriate regulatory standards.

  Participates in relevant Agency meetings and ongoing development of professional knowledge and skills.

  Actively supports the financial stability of the organization by meeting or exceeding established productivity and documentation standards.

  Informs supervisor of significant events/issues related to clients as appropriate and follows the program’s defined incident reporting process as required.

  Attends regular program and departmental meetings, case reviews, and CFC quarterly meetings.

  Maintains and develops professional knowledge by completing recommended education and training, workshops, conference attendance, and readings.

  Maintains knowledge of regulations and standards that govern client population.

  Participates in appropriate interagency meetings with other treatment agencies and community groups as required. Provide updates and communication with providers including those who cannot be represented in person.

  Participates in relevant Agency meetings and/or trainings

  Actively pursues the development of professional competencies related to job role performance through reading, supervision, in-service training, and attendance at conferences and workshops.

  Awareness and active support of the Agency's Mission, Vision, Values, and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  Qualifications

  Education: Bachelor’s degree in social work, human services, criminal justice, or related field preferred. An equivalent combination of education and experience will be considered.

  Credentials: Valid and clean NYS Driver’s License. Valid PPD screening is available.

  Experience: Minimum two years of experience with case management and community outreach in areas such as but not limited to persons affected by substance use disorder, prison re-entry, geriatrics, mental health, and refugees. Knowledge of HMIS, Medicaid, SNAP, Social Security, and legal systems is helpful. May require the performance of duties in various community locations and community settings that may not be handicapped accessible.

  Compliance: Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing, and documentation standards. All duties must be performed by CCFCS’s corporate compliance & ethics program.

  Willingness and ability to foster agency and program-wide cooperation and teamwork using positive/constructive communication techniques,

  Possession of a valid NYS Driver’s license and use of a registered and reliable vehicle.
