*Position Summary:*Youth Transition Coordinator
**This position will assist teens and young adults with disabilities, ages 14 to 22, to acquire knowledge, skills and supports necessary to prepare for a fulfilling, self-directed life after high school.
Services provided include information and referral, peer mentoring, advocacy, skills training, and other supportive services.This position requires consistent in-person presence in order to most effectively work with students, as well as to collaborate other team members and school educators.
**Primary Duties and Responsibilities:[Educate consumers ATTICs mission and services.Provide peer mentoring, IL skills training, information and referral, and advocacy to teens and young adults with disabilities individually or in groups.Provide workshops and presentations.Provides 1:1 or small group planning and/or instructional sessions. Manages group dynamics and ensures that environment and facilitation strategies are supportive to all learners.Performs other related duties as assigned.[Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections and serve on councils and boards as appropriate. Participate in collaborative community projects on disability issues.Work with other staff to create a network consisting of necessary resources to support consumers in their pursuit of independent living.Maintain an up-to-date expertise in disability rights legislation, regulations, and policies relevant to people with disabilities and advocate as appropriate.[Interact with other team members and Supervisor to provide comprehensive and timely assistance in resolving consumer issues, sharing information.[Assist consumers with development of their own Independent Living Plan (ILP) or Wavier with specific measurable goals to provide feedback for successful transitioning.[[]{style="
