About YDHP Housing Navigator

　　Work hours: Monday through Friday; flexible schedule with some work from home.Salary: $17.50 HourlyCredentials: Bachelor’s degree in social work, Psychology, Sociology, or Human Services with one (1) year post degree experience.Special skills/knowledge: Good communication skills, maintain client related goal data, including case notes, outreach, and relationships with community partners.Primary functions:Provide case management and navigation services designed to assist clients obtain housing.Develop and coordinate community wide outreach for youth homelessness.Develop plans of intervention that address needs for housing, services, crisis and intervention.Assist client with housing applications.Link clients to available resources to assist with meeting basic needs; Assist client with resolving identified barriers; Monitor clients’ needs and progress, Coordinate and facilitate with treatment team to assist client with reaching person centered goals.Network with agencies, coalitions, and local community meetings
