  Description

  * Up to $1,500 sign-on bonus for qualified new hires *(Paid out incrementally at 30 days, 90 days, and 6 months of employment)

  Title:WIC CPA or WIC Qualified Nutritionist, as appropriate to the level of education and/or experience

  Reports to: WIC Coordinator

  Rate of Pay:$22.00 - $23.54/hour

  Work Schedule:Monday - Friday, primarily day shift; the day-to-day schedule may vary slightly based on the needs of the individuals we serve. No evenings, weekends, or major holidays.

  Position Summary:Our team is seeking Specialist candidates to join our Women, Infants, and Children (WIC) team. There are currently openings in our St. Lawrence County and Franklin County, New York offices.

  This individual will support the mission, goals, and objectives of Community Health Center of the North Country by improving nutritional services in the WIC Program. Specialists will work with infants, children, and their caregivers, as well as pregnant, breastfeeding, and post-partum mothers. Specialists will be responsible for providing and documenting participant-centered nutrition assessments, education, and counseling individualized to the identified needs/interests of participants. Specialists will also receive cross-training to determine participants' eligibility for the WIC program.

  Responsibilities:(Please note the distinction between CPA and Qualified Nutritionist duties, where appropriate.)

  e. Demonstrates initiative for personal growth by attending in-services and meetings.

  Administrative/Clinic Dutiesa. Be cross-trained to cover all areas of clinic to avoid a gap in services and maintain clinic flow (ex. intake, height, weight, hemoglobin, answering phones, auto-dialer).

  b. Perform tasks to ensure participants are contacted with reminder calls, rescheduling no-show appointments and/or discharged according to policy.

  c. Assigned responsibilities, such as: Breastfeeding Coordinator, Voter Registration Coordinator, Outreach Coordinator, Workplace Committee, Safety Committee, van maintenance, clinic supervision, ordering supplies, and answering phones.

  Comply with program and agency policies and proceduresCPA ROLE:

  a. CPA will assess and counsel/educate any participants with basic nutritional needs.

  b. Conduct a complete nutrition assessment to determine nutritional risk and program eligibility and enroll participants.

  c. Prescribe, tailor, authorize, return, and reissue food packages.

  d. Issue food instruments with food package III once medical documentation has been obtained and approved.

  e. Identify and refer all high-risk participants to a Qualified Nutritionist.

  f. Work with Qualified Nutritionists to provide high-risk care to high-risk participants.

  g. Update and close high risk care plans in collaboration with a Qualified Nutritionist.

  h. Promote and provide education on the Farmers Market Nutrition Program.

  QUALIFIED NUTRITIONIST ROLE:

  a. Qualified Nutritionist will assess and counsel/educate any participants with basic nutritional needs but will focus on participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).

  b. Conduct a complete nutrition assessment to determine nutritional risk, and program eligibility and enroll participants.

  c. Develop and implement high-risk care plans for all high-risk participants.

  d. Provide oversight to CPAs in their follow-up of high-risk participants.

  e. Review and approve medical documentation for participants with Food Package III.

  f. Prescribe, tailor, authorize, return and reissue all food packages, including Food Package III.

  g. Promote and provide education on the Farmers Market Nutrition Program

  Core ServicesCPA ROLE:

  a. Provide and document any participant-centered nutrition education (individual/family sessions or facilitated group discussions) responsive to the identified needs/interests of participants.

  b. CPA will facilitate groups as needed.

  c. Support participants in their decision to breastfeed, refer to Enhanced Peer Counselor Program and assist in providing a breastfeeding-friendly environment.

  d. Perform hematology testing and anthropometry as needed.

  e. Responsible for promoting extended hours, including lunchtime appointments, in order to increase access to working participants.

  QUALIFIED NUTRITIONIST ROLE:

  a. Provide leadership in the planning, conducting, and evaluation of participant-centered nutrition education (individual/family sessions or facilitated group discussions) responsive to the identified needs/interests of participants.

  b. Qualified Nutritionist will facilitate groups as needed.

  c. Promote healthy lifestyle plan, including physical activity options in the clinic and community.

  d. Support participants in their decision to breastfeed, refer to Enhanced Peer Counselor Program, and assist in providing a breastfeeding-friendly environment.

  e. Perform hematology testing and anthropometry as needed.

  f. Responsible for promoting extended hours, including lunchtime appointments, in order to increase access to working participants.

  DocumentationCPA ROLE:

  a. Provide and document appropriate follow-up referrals in NYWIC to other health and social service agencies for participants, especially participants with additional nutrition or health concerns.

  b. Document all individual and group contacts in NYWIC.

  c. Implementoptionalcare plans for participants, especially participants with additional nutrition or health concerns.

  d. Update high risk care plans once established by the Qualified Nutritionist.

  e. Track participants’ progress, including goals set to improve health and document outcomes.

  f. Indicate the requirements for participant’s next appointment (ex. high-risk, immunizations, height, weight, hemoglobin) in the participant’s record.

  QUALIFIED NUTRITIONIST ROLE:

  a. Document any participant-centered nutrition education (individual/family sessions or facilitated group discussions) responsive to the identified needs/interests of participants, especially those participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).

  b. Provide and document appropriate follow-up referrals in NYWIC to other health and social service agencies for participants, especially participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).

  c. Implement optional and required individual care plans for participants, especially participants whose nutrition or health concerns require an advanced level of nutrition expertise (high risk).

  d. Track participants’ progress, including goals set to improve health and document outcomes, especially participants whose additional nutrition or health concerns require an advanced level of nutrition expertise (high risk).

  e. Indicate the requirements for the participant’s next appointment (ex., high-risk counseling, immunizations, height, weight, hemoglobin) in the participant’s record.

  Training and Quality AssuranceCPA ROLE:

  a. Attend and participate in all mandated trainings.

  b. Obtain 12 hours of continuing education every year in topics relevant to WIC.

  c. Keep documentation of all trainings up to date per Local Agency Policy.

  d. Perform quality assurance as instructed by the WIC Coordinator.

  e. Review active and discharge charts per QA policy.

  QUALIFIED NUTRITIONIST ROLE:

  a. Attend and participate in all mandated trainings.

  b. Obtain 24 hours of continuing education every 3 years in topics relevant to WIC.

  c. Keep documentation of all trainings up to date per Local Agency Policy.

  d. Provide leadership in developing and conducting nutrition training of LA staff in nutrition education topics.

  e. Perform quality assurance as instructed by WIC Coordinator.

  f. Review active and discharge charts per QA policy.

  g. Provide leadership in orienting/training/mentoring CPAs and nutrition support staff.

  Outreacha. Perform Public Health Detailing as directed by WIC Coordinator/Outreach Coordinator.

  b. Participate in outreach activities to expand caseload and agency visibility.

  Communicationsa. Responsible for reviewing all NYS DOH memos/WIC Watch/policies available in NYWIC.

  b. Responsible for accessing Community Health Center of the North Country emails and responding accordingly.

  c. Maintain Community Health Center of the North Country updates via agency newsletter and memos.

  d. Responsible for all information provided at staff meeting (if unable to attend, obtain from co-worker or meeting minutes).

  e. Keep open communication with co-workers, WIC Coordinator, and Nutrition Coordinator to improve workflow on a daily basis.

  Temp. Clinic Responsibilitiesa. Prepare supply boxes for clinic.

  b. Lifting and carrying supply boxes, when necessary.

  c. Safe lifting and carrying of heavy equipment.

  d. Assist in the transportation of staff and supplies to WIC clinics.

  e. Set-up and tear-down clinics.

  Responsible for neatness of office and clinic areas.

  Perform related duties as assigned.

  Requirements

  Candidates must have flexibility in available work hours and be available to travel daily within and between St. Lawrence and Franklin Counties. Candidates must have a valid driver’s license.

  Opportunities are available for candidates withat least oneof the following educational requirements:

  Bachelor’s degree in an appropriate health-related field with at least six (6) credit hours in nutrition*,OR

  Nutrition and Dietetics Technician, Registered (DTR/NDTR),OR

  Registered Nurse (RN) with Bachelor’s degree,OR

  Registered Dietitian Nutritionist (RD/RDN),OR

  Bachelor’s, Master’s, or Doctoral degree in Nutrition

  *For the otherwise-qualified candidate, up to three (3) of the required six (6) hours of nutrition education may be taken within six (6) months from the first date of employment.
