*Summary: *\Provide Supportive Services to Veterans and their families in this grantfunded program. Provide case management services to Veterans to developand execute Individual Housing Plans that enhance housing security toeligible participants through direct and indirect service provision inGoodwill and partner agencies. Must have valid driver license withpersonal auto insurance as well as pass background and driver recordcheck.\
Education and/or Experience:
Bachelor\'s degree (B. A.) from four-year college or university; or oneto two years related experience and/or training; or equivalentcombination of education and experience.
*Essential Duties and Responsibilities include the following: *
Effectively assist participants to achieve and maintain housingstability by coordinating and/or providing supportive services.
Ensure that participants receive the assistance needed to promotehousing stability, including but not limited to: consulting withparticipants to conduct needs assessment, working with participants tocreate individualized housing stability plans that include specificplans and specific goals, monitoring participants from the time ofintake until they exit the program, and ensuring that participantsobtain needed supportive services, re-evaluate participant eligibility,per program requirements.
Interpret and explain information, such as eligibility requirements,application details, and program specific information.
Determine the participant\'s household needs and priorities. Gather andrecord information about the participant\'s health, income, eligibilityfor public benefits, employment skills, background, family relationshipsand supports, and living situation in order to identify and coordinatethose services that will be most effective and acceptable to theparticipant in improving housing stability.
Coordinate activities and work collaboratively with department andcontracted community agencies providing services under the SSVF programto ensure that Individual Housing Plans are achieved.
Create and utilize informal community networks, as needed, to locateresources and make referrals as necessary in support of IndividualHousing Plans.
Participate in meetings, staffings, and other activities as directed topromote and review the effectiveness of Individual Housing Plans andoverall case management efforts.
Conduct other services and activities in concert with Department ofVeterans Affairs, Supportive Services for Veterans Families programguide and at the direction of Program Manager.
Establish and maintain good working relationships with participants,referral agencies, community partners, and all team members.
Ensure all paperwork and related activities are entered, current,accurate, and in compliance in a timely manner. Revise as necessary.\
Seek new sources of revenue that will meet Goodwill\'s mission and alignwith strategic plan, upon request.
Provide services in accordance with Goodwill policies, procedures, CARFstandards, and principles of safety for personnel, participants andproperty.
