CHI Health has an immediate need to add a Triage Clinic RN to our Millard Family Clinic located off of 153rd Q!

Schedule: Monday-Friday 8a-430p

Our full-service network provides a variety of Primary Care Priority Care Services, including Family, Internal, Geriatric, and Pediatric Medicine, in addition to several specialties to deliver custom care based on the unique needs of our patients.

Responsibilities

This position will support our healthcare teams by ensuring they have everything they need in order to care for our patients. They also play a huge role in a patient’s experience and prep when they visit any of our clinics. We couldn’t do it without you! Expectations:

Perform telephone triage for patients calling their provider with certain symptomatology.

Demonstrates the ability to use the nursing process to collect data. Requires interview/communication skills, critical thinking and problem solving skills to level of licensure.

Ability to work within protocols approved by CHI Health to provide information to patients and consumers.

Accepts inbound calls from patients, pharmacies and other healthcare entities.

Performs outbound calls to notify patients of reviewed lab and imaging results according to protocols that require review of the patient chart to answer questions

Performs general nursing care to patients by following established clinic workflows.

May perform specific nursing care as it relates to the specialty of the practice.

Obtain patient consent for care and ensure patient confidentiality.

Obtain and document health history, information on chief complaint, vital signs, and health risk factors and assess disease conditions.

Provide patient care based on practice guidelines, standards of care, and federal/state laws and regulations.

Execute prescribed treatments and medical interventions, administer prescribed medications, and monitors and documents treatment progress and patient response. Refer to/consult with physicians, other health providers, and community resources to prevent/resolve problems or concerns.

Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.

Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.

Participate in multidisciplinary teams to improve patient care processes and outcomes.

Track quality assurance data and monitor for acute and chronic care management.

May act as team lead for other nursing staff.

Performs other duties within scope of role as assigned.

Qualifications

Minimum Qualifications:

Current Registered Nurse (RN) license that allows you to practice in our state

Graduate of an accredited nursing program

Basic Life Support (BLS) from the American Heart Association required

Pay Range

$26.38 - $38.26 /hour
