Duties include transporting residents to and from appointments.

　　Must have a current drivers license in good standing

　　Must be flexible with hours.

　　Must be able to operate a Bus with lift, car and Durango

For what reason and time limit such as within one or || two days or weeks. || ||              .      Transporters needs to make sure that you take || Resident face sheet, medication list, History and Physical (if || needed) and resident referral form. || ||              .      Transporters needs to check with Business Office || Manager regarding payor source and what appointments are being made || during residents stay at the facililty prior to the transportation of || the resident.  || ';   ||                                                                       || ||             .      Transporter needs to have patient sheet with payor || source on and an understanding of Medicare and Medicaid guidelines || regarding pt. benefits. || ||       |+-----------------------------------------------------------------------+
