Position Description Program: TransitionsClinic/Progressive Community Health Centers Position: Peer SpecialistPosition Summary:The Transitions Clinic Peer Specialist (TCN P.S.) willprovide peer support, navigation, and support services to peoplerecently re-entering the community who are interested in establishingcare at Progressive Community Health Centers (PCHC). These individualsmay be impacted by a significant medical, substance use and/orbehavioral health needs which can be successfully managed in PCHC\'shealth home. The TCN Peer Specialist contributes to an interdisciplinaryhealth care team and documents interactions in the electronic medicalrecord. Additionally, the TCN Peer Specialist will coordinate outreachactivities in the community, correctional settings, or local jails. APeer Specialist is a person who has lived the experience of mentalhealth, substance use or co-occurring needs, is in recovery, has hadformal training in the peer specialist model of support, and has passedthe State of Wisconsin certification exam or successfully becomescertified within one year of hire. The Peer Specialist in the TCNProgram would benefit from having lived experience in the department ofcorrections or justice system. The Peer Specialists utilize their ownunique lived experience to engage the people they are serving and rolemodel recovery during each interaction. Essential Functions: Provideindividualized peer support and navigation through systems of care(e.g.assist with establishing a medical home, scheduling appointments,offering reminders, providing self-management support, deliveringeducation, and assisting individuals in finding answers to questionsthey may have about their physical and mental health needs). Promotecollaboration between the individual and PCHC\'s medical and behavioralhealth providers. Provide 1:1 peer support services to people in thecommunity (including their home and other community-based locations).From a recovery perspective, provide a strength-based review of anindividual\'s abilities, strengths, and needs.
