　　We are seeking a Behavioral Health Consultant (BHC) / OP Therapist (LCSW Required) to join our team at our program in Philadelphia, PA 19154.

　　Earn $70,000/year.Plus, there is a $2,000 SIGN-ON BONUS!

　　Position DetailsThe Behavioral Health Consultant (BHC) - OP Therapist (LCSW Required) will provide comprehensive assessment and diagnosis of behavioral health clients with effective treatment planning and assisting clients in successfully achieving goals.  You will evaluate crisis situations and apply appropriate interventions, actively participate in meetings that support Merakey integrated health care model to provide comprehensive care for clients.  You will assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease and provide assessment, consultation, and brief intervention for psychological/ psychiatric problems and/or disorders.  Additional responsibilities will include teaching patients, families, and staff care, prevention, and treatment enhancement techniques, monitor the site's behavioral health program, identifying problems related to patient services and making recommendations for improvement, participate in development and administration of data and measures to assess multiple outcomes, to include patient and provider satisfaction, institutional and provider fidelity to the model, patient health outcomes, etc. Therapist will have 2 days a week on site work as a BHC & 3 days a week carrying a caseload of Outpatient Clients at the Knights Rd Location (hybrid work available for these 3 days).

　　Requirements

　　The ideal candidate will possess the following qualifications.Master's Degree in Social Work.Licensed in the State of Pennsylvania as a Licensed Clinical Social Worker (LCSW) or a PA Licensed Psychologist.One year of experience in a therapist role working with children preferred.Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.
