HANAC, Inc. – originally founded by George Douris as the Hellenic American Neighborhood Action Committee – is a New York City-based multi-faceted social services organization. The organization was founded in 1972 to serve the needs of vulnerable populations throughout New York City.
The Tenant Coordinator will provide social services for tenants in HPD's Senior Affordable Rental Apartments (SARA) Program in Boston Rd Clinton. The tenant coordinator will work closely with the Program Director, Social Work Supervisor, and another Tenant Coordinator to develop and utilize best practices to assist tenants in 67 apartment units ages 60+, some of whom were previously homeless. The tenant coordinator will be responsible for all aspects of case assistance including ongoing care plans, managing and following through with referrals, developing relationships with collateral agency providers, and assisting clients with remaining permanently housed as well as having access to optimum adaptive solutions to age in place. The Tenant Coordinator will also be responsible for implementing programming and activities to create a positive, inclusive community environment.
Rate of Pay: $25 per hour
Schedules: You should be able to work one of the following schedules below
Schedule: T-W-TH-F 10:30 am to 6:00pm 7hrs a day X 4 Days a week= 28hrs a week
Responsibilities:
Maintain regular contact with tenants on caseload to assess their needs and assist where required
Develop individual care plans in coordination with the tenant’s goals and update a minimum of every 6 months
Coordinate case assistance needs, including applying/maintaining entitlements and benefits, healthcare coordination when necessary, and assistance with maintaining housing
Maintain good rapport with Property Management in order to best assist tenants when issues arise
Build relationships with community providers; provide advocacy and resource linkages/referrals as required
Work with service providers and family members to build networks of supports
Plan activities and events to foster a sense of community among tenants including, celebrations, community provider presentations, training, arts and crafts, bingo, etc.
Keep tenants informed of Tenant Coordinator availability and all activities/events to take place
Completion of Initial Intake, assessment, and file consent form in a timely manner when new tenants move in
Conduct periodic home visits/wellness checks for tenants with special needs or circumstances
Maintain daily case assistance log, complete detailed and accurate progress notes in a timely manner, complete detailed monthly report for HPD in a timely manner, and attend required training
Data collection when necessary to determine the needs of the program
Assist Supervisor and/or Program Director as needed and in preparation for bi-annual audits
Qualifications and Experience:
Bachelor’s Degree in Social Work, or related field; or two years of college and demonstrated experience working with aging populations.
Bilingual preferred, but not required (Spanish, Chinese, or Korean preferred).
Knowledgeable in benefits and entitlements for low-income senior residents, including Section 8, Medicaid, and SNAP benefits.
Microsoft Office proficient
Must be flexible, able to work independently, responsible, reliable, and able to work as a team member contributing to a positive work environment. The ideal candidate will be able to handle a crisis and implement problem-solving skills
Must be people-orientated, professional, and courteous with excellent phone etiquette.
