CAREER CENTER SUMMARY Services include individual and team case management, Resource Center access and support, workshops and other partner and community based services aimed at developing job seekers skills for employment success. The Supportive Services Coordinator is a member of the CNY Works Career Centers Adult and Dislocated Worker Workforce Services cross functional team responsible for ensuring that Quality Jobs, Equity, Strategy, and Training QUEST National Dislocated Worker Grant NDWG participants receive comprehensive supportive services. Core functions include developing customers Individual Service Strategy ISS documents by conducting comprehensive assessments, assisting with direct service eligibility, application, and referral processes, and coordinating care and case management. The Supportive Services Coordinators primary role is to function as the key direct service contact for QUEST eligible dislocated workers to connect them with established, trusted community service providers. Delivers follow up services, tracks, and analyzes data on an individual, caseload, and agency wide basis. Meets reporting, performance, and outcomes goals as required by NYSDOL. Evaluates and assesses each QUEST program participant using NYSDOL Form ES102 career Services Eligibility to identify employment barriers. Creates Individual Service Strategy ISS for each QUEST participant requiring supportive services only based on NYSDOL Form ES102 to coordinate services, care, and referrals to support each participants needs and monitors ongoing progress toward self sufficiency.Connects and supports participants in accordance with their ISS in obtaining all entitlement program benefits, including but not limited to Child Care Assistance Program CCAP, Home Energy Assistance Program HEAP, Supplemental Nutrition Assistance Program SNAP, Temporary Assistance to Needy Families TANF, Women, Infants, and Children WIC, etc. Maintains accurate and comprehensive files, ensuring compliance with confidentiality and audit standards. Administers CNY Works, Inc. QUEST NDWG Supportive Services and Needs Related Payment policies, including completing CNY Works QUEST NDWG Supportive Services Justification Forms in coordination with the customers assigned Workforce Advisor to include required costs and payment documentation, for approval by Adult and Dislocated Worker Workforce Services Director. Upon approval, coordinates with CNY Works Finance Team in accordance with CNY Works Procurement Policy, to complete approved vendor coordination and purchase order processing submissions. Tracks all CNY Works QUEST NDWG Supportive Services Justification Forms submissions, approvals, costs, and payment requirements in coordination with the Adult and Dislocated Worker Workforce Services Director and CNY Works Finance Team. Monitors and conducts ISS follow up at all points during QUEST program participation to support participant job and work readiness development via occupational skills and work readiness training to mitigate employment barriers to ensure that the participant is a hire ready viable job candidate prepared for job placement. Develops Reporting,
