　　As a Supervisor on the Scripps Green team, you will coach and develop staff to ensure the hospitalneeds are met byfacilitating a safe discharge plan for our patients by removing psycho-social barriers to discharge.

　　Other duties include;

　　· Providing/overseeing psychosocial evaluation/consultation/referral and triage of assigned patients.

　　· Conducting clinical assessments of patient/family/total situation to establish intervention strategies.

　　· Developing resources such as educational programs for community agencies or hospital or ambulatory personnel.

　　· Facilitating patient care conferences with other team members to review patient's goals of care, provide psychosocial support, assist with advance care planning and ensure proper referrals are made to community resources.

　　· Ensuring timely and effective transfer between facilities and nursing units; also facilitating transfer to appropriate level of care in order to ensure quality care, maximum reimbursement, and minimizing cost/day.

　　· Conducting or attending discharge planning conferences with interdisciplinary healthcare team members.

　　· Supervising other members of the social work team in providing the same level of discharge planning Provides clinical direction.

　　· Facilitating and directing peer group supervision with interdisciplinary clinical team.

　　· Providing and/or coordinating in-service education and training for staff.

　　This is a Full Time, 8-hour day shift position.

　　Required Qualifications:

　　· Master’s degree in Social Work (MSW)

　　· LCSW * BLS for Healthcare Provider American Heart Association * Working knowledge of the laws, regulations, policies and appeal procedures as relative to the Federal, State and other reimbursement programs * Recent experience in acute care hospital as a Medical Social Worker, including knowledge and proficiency of discharge planning * Must be able to display good working relationships with patients, families, medical and nursing personnel, and all others employed at the hospital

　　Preferred Qualifications: * Proficiency in Microsoft Office including but not limited to Outlook, ExcelEpic, ECIN. * An ability to speak another language is desirable.
