About Sr. Project Manager / Health Equity Clinical Manager

  GENERAL PURPOSE:

  Serves as the primary leader for Cardiac Care Health Equity initiatives for operational improvements and optimization along with new program and service development and deployment of innovative ways to bring care delivery systems to underserved areas. Works closely with the Director of Operations, Chief of Cardiology and Cardiology Leadership on projects and initiatives that drive better care and transform care with innovation and technology. Assist in project implementation of grants and trials in building new programs and integrating them into Cardiac Care model along with enhancing current workflows and practices. Utilizes data and dashboards for all of general cardiology, cardiac surgery and sub-specialties to improve clinical, operational and financial outcomes for the Health Equity initiatives in the service line. Incorporates the organizational lean operational principles to apply the most streamlined team approach to outcomes.

  JOB DUTIES AND RESPONSIBILITIES:

  Utilizes data sources to analyze and identify opportunities for improvements. Assists in team development and initiatives to create outcome driven projects looking to enhance current operational, clinical, financial programs within Cardiac Care or create and integrate new services and offerings based on research and innovations in Cardiac Care specifically directed at Health Equity initiatives driven from the division. Collaborates across teams and divisions connected to initiatives and shared resources.

  Works with a high degree of independence and organizational skill to develop work-plans and goals with teams based on Operational Excellence and Quality principles to drive better operational and clinical outcomes. Able to lead teams through processes of change management and facilitate change in positive and goal directed manner with large and small teams. Initiative to constantly look for process and clinic changes to meet the health needs of patients.

  Strong Analytical skills and experience in data analysis. Strong abilities to use Excel and organizing data into PowerPoints and executive dashboards Excellence in clinical data evaluation and troubleshooting for erroneous or error in order to ensure high quality data for projects and outcomes. Strong ability to analyze, troubleshoot and make recommendations for optimal outcomes based on data and team group. Strong leadership and collaborative skills to lead projects. Integrate data and feedback to alter operations and ensure change in clinical delivery models to meet unique health equity goals

  Assist in the development for collection and analysis of data by the use of questionnaires, interviews, reports and searches of existing journals, etc. Strong skillset in analyzing operational data and financial data and able to work with teams in ensuring valid data and the use of data to transform into usable operation information and measurable metrics.

  Provide Cardiac Clinical expertise and leadership in designing new clinic operations, policies and protocols to encourage compliance in health equity initiatives with patients. Manages and develops training for staff involved in initiatives that are unique to the clinical experience. Sets goals and provides recommendations to clinical teams and support teams on direction to enhance care and processes related to care. Works collaborative across departments that are integral to cardiac care in ensuring project and patient flow initiatives cross all care centers for the best patient flow. Strong understanding in patient flow through multiple clinical setting and ability to map out and work with teams in flow analysis to collaborate best practice solutions.

  Management and Performance responsibility for reports performing similar work, and supervises clerical staff and field work aides. Works closely with technical team members to create usable dashboard for operational excellence and growth management. Assists Director of Operations in projects to increase manager’s abilities across the cardiac care division to use clinical recommendations in cardiac operations. Assist Director of Operations in Cardiac Care Lean initiatives to improve operational efficiency and staffing measures.

  Other duties as assigned

  QUALIFICATIONS:

  Bachelor’s Degree preferred

  Graduate of accredited school of nursing/Licensed in New York State as a Registered Nurse

  Preferred two (2) years of cardiovascular or Critical Care experience. Data collection and analysis preferred, but not required.

  Preferred Master Degree

  Preferred two (2) years of cardiovascular or Critical Care experience. Data collection and analysis preferred, but not required.

  Or equivalent combination of education and experience required

  Detail oriented with strong organizational and prioritization skills and ability to work independently under general directions preferred.

  Ability to interact with all members of Cardiac Care Service Line including Champions, Physicians, and administrative staff in a professional and courteous manner preferred.

  Proficiency in Microsoft Office (Word, Excel, PowerPoint, Access), software/web applications, and electronic medical record preferred.

  Clinical experience preferred.

  Statistical knowledge preferred

  Quality Assurance and Process Improvement (QA/PI) experience preferred

  Pay Range

  Pay Range: $68,765 - $103,147 Annually

  The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

  Location: Strong Memorial Hospital

  Full/Part Time: Full-Time

  Opening: Full Time 40 hours Grade 055 CVSL Outpatient ACG & CC

  Schedule: 8 AM-4:30 PM
