GENERAL PURPOSE

　　Performs evaluations and develops effective patient treatment plans to restore, maintain or prevent decline of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing outpatient and home health care. Services may be performed in therapy clinic or individual patient residence.

　　Position: Speech Language Pathologist - SLP

　　Hours: no evenings or weekends or holiday. Monday thru Friday, CONSISTENT PRN and Part time (20-25) hours weekly will be considered

　　Setting: Assisted Living Community

　　Location: Rose Arbor and Wildflower Lodge, Maple Grove, M

　　QUALIFICATIONS

　　Graduate from Speech-Language Pathology program

　　Current license or ability to obtain as Speech-Language Pathologist in the state of practice

　　Previous experience in outpatient, preferred

　　Must meet Clinical Competency requirements

　　Current CPR certification, if practicing in home health
