　　THE POSITION

　　Assists in meeting the psychosocial needs of patients in order for patients and families to achieve their optimal level of health. Responsible for providing discharge planning services based on assessment and treatment team recommendation. Meets the post-discharge needs of the patient and family through linkage with community resources including long-term care facilities, assisted living facilities, Home Health agencies, equipment companies, etc. Completes appropriate transfer forms such as FL-2s and PASARR’s. Provides discharge planning assessments and coordinates with the multidisciplinary treatment team to ensure psychosocial needs are met. Works under the direction of the Director/Manager of Case Management. Insures effective utilization of the health care system for the benefit of the patient. Performs other related duties as required and deemed competent. Bachelor of Arts (Accredited School of Social Work preferred, other Human Service degrees considered) or combination of experience and education. 1 year in a health care setting preferred.
