The Prevention Service Area of Health and Human Services seeks several Senior Social Workers for the Parent Support Outreach (PSOP) team. PSOP workers provide voluntary child welfare services to families who have risk factors of child abuse and neglect. Location and hours: These positions are hybrid and will be performed both on-site at various locations throughout Hennepin County and remote as job duties require. PSOP workers are required to meet face-to-face with participating families at least once a month both in the community and in their homes. Work hours are typically Monday through Friday, 8 a.m. to 4:30 p.m., with occasional evenings or weekends to accommodate participants' scheduling needs. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. In these positions you will: Engage with participants in voluntary services and provide strengths-based case management. Complete referrals to community agencies to address long-term needs. Communicate with internal and external stakeholders via phone, email, and in-person in accordance with the case management role. Complete documentation in accordance with program standards. Conduct in-person visits in participant homes and various other community locations.
