About Social Worker (Program Coordinator) COMPACT ACT

  Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Duties include, but not limited to: Serves as clinical care team point of contact and facility subject matter expert on COMPACT Act and provides clinical case management to Veterans who are actively involved in utilizing COMPACT Act services. Responsible for evaluating implementation efforts at the facility and VISN, if requested, and for ensuring adherence to policies and procedures from the OMHSP. Uses a high level of skill in assessing and treating the complicated mental health and psychosocial problems of Veterans and their families/significant others, including case management of highly complex cases. Works frequently with Veterans that may have complex medical and psychiatric needs. Support Veteran's recovery, goals, and preferences for mental health/medical care. Engage in discussions with the Veteran (and caregiver if available) regarding warning symptoms or signs to monitor that may indicate that the Veteran's condition has worsened and who to contact in an emergency. Introduce efforts to reduce non-VA inpatient facility readmissions and length of stay to support Veterans in the least restrictive environment and cost-savings. Serve as a role model for utilization of appropriate communication and conflict management techniques, effectively guiding and motivating team members. Evaluate practice for a service, based on established professional, community and regulatory standards and recommend/implement changes to meet those standards. Act as liaison between providers, team members, VA and community resources for continuity of care to include identification of need of care at home or alternatives such as outpatient treatment, extended care facility and/or assisted living facility. Understand the complexities of bureaucracy and acts as an advocate within the VA system when it serves the best interests of the Veteran/family. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience interfacing with stakeholders across the healthcare system to include community entities. Experience with issues surrounding Eligibility, billing, and suicide prevention activities. References: VA Handbook 5005 Appendix G39 The full performance level of this vacancy is GS-12. Physical Requirements: Physical aspects associated with work required of this assignment are typical for the occupation, see Duties section for essential job duties of the position. May require standing, lifting, carrying, sitting, stooping, bending, pulling, and pushing. May be required to wear personal protective equipment and undergo annual TB screening or testing as conditions of employment. Driver's license and physical exam required to confirm ability to operate a government vehicle. Additional Information Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. This credited service can be used in determining the rate at which they earn annual leave. Such credit must be requested and approved prior to the appointment date and is not guaranteed.
