JOB REQUIREMENTS: Overview Provides counseling, information, anddirection to obtain community resources when needed for patients of thehome health and hospice agency and their families, while collaboratingwith physicians, nurses and the allied health team according to the planof care by performing the following duties: Responsibilities Performshome visits to patient\'s dwelling. Investigates case situations andpresents information to members of the health team on patient\'s familyand social background pertinent to diagnosis and treatment. Assesspatients for continuing care needs, creates plan for providingcontinuing care, and confers with physician and team members as needed.Refers patient or patient\'s family to other community resources as isindicated; assists patients and families to complete advance directives.Educates patients about resources, options and documents all activitiesin a timely manner. The MSW provides emotional support, helps patientsand families understand and accept their condition as well as acceptingmedical recommendations; utilizes community resources to assist patientsto live in the community; responsible for travel arrangements forpatients; responsible for arranging changes in levels of care andsettings; The hospice MSW participates in IDT meetings and is considereda core member of the Hospice team per regulations. The home health MSWparticipates in team meetings regularly to provide input during patientcase conferences and to educate team members regarding social workinterventions that can be provided. May be required to work flexiblehours. Education and Experience Masters of Social Work (MSW) from anapproved graduate school of social work. Home health or hospiceexperience desired. One year social work experience in a healthcaresetting. Experience and ability to facilitate groups.Certification/Licensure Currently Licensed (LCSW/LSW) in the State ofIllinois and/or Wisconsin Licensed to practice in the State of Wisconsinand/or Illinois or registry eligible Approved CPR or BLS certificationrequired within 90 days (Mercyhealth at Home Only) - For those drivingMercyhealth vehicles: Maintain a valid driver\'s license. Have andmaintain an acceptable driving record per Mercyhealth\'s HR.100 Fleetand Driver Safety policy.
