The Social Worker MSW is a graduate level professional that has demonstrated expertise in the assessment and treatment of patients in a variety of inpatient and outpatient practice settings. Facilitates individualized, goal directed discharge processes in accordance with departmental hospital policies and procedures. Performs complex bio-psychosocial assessments and case formulations, including family assessments, and to execute timely and effective interventions, in collaboration with the Health Care Team.
