The Social Worker MSW/LSW or LCSWis astrong and collaborative team member who provides psychosocial patient assessments and supportive intervention, and develops plans for safe transitions from the hospital using an interdisciplinary approach. Manages complex social and discharge issues, builds and maintains relationships with internal/external resources, and links patients to community agencies and entitlement programs to address gaps in social determinant of health. Demonstrates an understanding of all stages of growth and development and ability to assess each patient's age-specific and cultural needs to provide individual treatment and care.
Position requirements:
Masters of Social Work degree.
NJ State license required by NJ State Board of Social Work Examiners. Licensed Social Worker (LSW), or licensed clinical social worker (LCSW) required.
Recent Master s degree candidates who have successfully passed the licensure exam but are waiting for license are welcome to apply now.
Minimally one year of experience in an acute hospital setting.
Will consider a full year hospital internship.
