Position Summary:
The Social Worker acts in a consultant capacity within the multidisciplinary care team and facilitates psychosocial screening with members who are referred by case management programs. They collaborate with the care team (CMs, physicians, nurses, community agencies, etc.) to manage members' psychosocial and behavioral issues. They also execute on many of the psychosocial aspects of the care plan with the primary care team (CM and physician).
Qualifications:
EDUCATIONAL REQUIREMENTS Master's Degree in Social Work or Bachelor's Degree in Social Work CERTIFICATION & LICENSURE REQUIREMENTS Licensed Clinical Social Worker (LCSW) Illinois upon hire or Licensed Social Worker (LSW) Illinois upon hire. EXPERIENCE REQUIREMENTS Two (2) years related experience. SKILLS AND KNOWLEDGE Ability to problem solve and develop solutions for members and their families, strong knowledge of available community resources. Awareness of when and how to access additional resources to meet needs of members. Strong advocacy skills necessary.
Essential Functions:
Can service as primary care coordinator when member needs require the soical work expertise.
Works with Care Coordinators and CTI Care Coordinators to manage socioeconomic and psychosocial support as needed
Conducts socioeconomic and psychosocial evaluation.
Recommends community or network resources to which members can be referred or steered
Organizes community resources for members through contact and negotiations with external providers of these services.
Facilitates end-of-life discussions, transfers, and placement.
