　　Job Summary:

　　As a social worker at Trinitas Regional Medical Center/RWJBarnabas Health, you will provide front-line services to patients with conditions spanning the entire healthcare continuum. Understands the financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication.

　　Collaborates closely with physician advisors, participates in interdisciplinary rounds, and facilitates communication with the interdisciplinary care team. Complete psychosocial assessments and implement patient-centered care plans that meet the needs identified in the assessments.

　　Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF/ rehab placements, hospice, community resources, and transportation arrangements. Collaborates with RN Case Manager in all discharge planning,

　　Collaborate with department leadership to streamline processes and implement strategies to create more efficient delivery of care,

　　Ensures compliance with regulatory documentation standards.

　　EDUCATION:

　　Graduation from an accredited graduate school of Social Work with a Master's in Social Work,

　　REQUIREMENTS:

　　State licensure is required by the NJ State Board of Social Work Examiners.

　　Licensed social worker or licensed clinical social worker depending upon years of social work experience and clinical coursework,

　　Successful completion of all orientation programs
