Citizens Memorial Hospital (CMH)is seeking aSocial Workerfor the position of Pre-Natal Care Coordinatorat our OBGYN Clinic.

　　Typical duties will include:

　　Completingscreenings and assessments to determine needs.Facilitatingaccess to pre-natal care and linkage to services.Providing client education, andassisting with resources to be successful at home.Maintaining accurate documentation and submitting all forms and paperwork in a timely manner.Conducting work tasks in a manner consistent with the mission and vision of CMH.This position is a 5-year, grant-funded position. The schedule will be full-time, Monday - Friday, 8am-5pm.

　　A bachelor's degree in social work or a related field is required.
