　　Job Summary:  The Social Worker will provide direct casework and counseling services to the patient/family as directed by the plan of care.  The Social Worker will also provide psychosocial assessments and develops a corresponding care plan as coordinated with the Interdisciplinary Team, IDT.

　　Job Description:

　　Provide psychosocial assessments, direct casework and counseling services to patients and their families.

　　Participate in the preparation, evaluation and execution of the interdisciplinary care plan to assure that the aims for continuity and comprehensiveness of care are accomplished.

　　Work collaboratively as a member of the Interdisciplinary Team(s).

　　Demonstrate understanding and adherence to professional Codes of Ethics and related organizational policies.

　　Maintain and update knowledge of community resources.   Facilitate referral of patients/families to those resources as needed and desired.

　　Qualifications:

　　Master’s degree in social work from an accredited educational institution or other related clinical degree

　　Minimum two (2) years in clinical social work required; one (1) year experience in health care preferred.

　　Understanding of psychosocial factors operative in terminally ill patients and families required

　　Knowledge of the process of death, dying and bereavement required. 

　　LSW required

　　Strong interpersonal and communication skills required.

　　Ability to travel within the State of NJ
