DescriptionPosition Summary/Essential Duties:Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital.Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).Provides age specific care to patients/families in all age groups.Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.Develops plans with attention to individual patient and family goalsProvides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.Communicates/collaborates with the work team using a multidisciplinary approach.Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.Assists with recruitment, and orientation/mentoring/education of new staff and or social work interns.Equal Opportunity Employer -- minorities/females/veterans/individuals with disabilities/sexual orientation/gender identity
