Social Service Facilitator (Virtual/Remote/Community)Several Full and Part-Time positions available

　　Monday through Saturday, Hours are flexible based on client need.Transitions Thru Life, Inc. located in Benwood, WV is looking for several energetic individuals to fill the Social Service Facilitator position who will provide consistent high quality case management, clinical treatment, advocacy, and community referrals as needed by the client. The Social Service Facilitator will also engage in the multidisciplinary team.

　　Qualifications:

　　Candidate must have a Bachelor's degree. For an applicant with a master's applying for the SSF position, they do not need to be licensed.  

　　Applicants pending the completion of licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

　　Prior experience with SAH, WRAP, DHHR, and courts preferred.

　　Job Duties Include:

　　Screening & Assessment, Treatment Planning, Community Linkage, Resource Development, Advocacy, Education & other duties as assigned.
