Do you like working in a fast-paced environment? The Family Network is hiring for a Service Coordinator. The Service Coordinator serves as a liaison between DCF and outside providers. Responsible for identifying, coordinating and monitoring community-based services for families and children.
How You Will Be Making a Difference
Select service models, providers and community resources appropriate to the needs of the youth and family.
Identify and coordinate the intensity and frequency of services
Coordinate any changes in service providers
Manage duration and termination of services based on outcomes
Participate in case management team meetings, school meetings and other provider meetings necessary.
Ensure that referral packets for services are complete and sent out in a timely manner.
Qualifications
Minimum of Bachelor’s Degree.
Knowledge of Department of Children and Families, Department of Early Education and Care, Department of Mental Health and other state systems, and community resources.
Must have driver’s license as frequent travel is required. Varied work hours to be expected.
Proficient computer skills.
Excellent written and verbal communication skills.
