　　Job Summary and Qualifications

　　The Sepsis Coordinator us responsible for the implementation and coordination of a program that delivers evidence based care care to sepsis patients across the continuum of care. Coordinates tasks and activities related to the hospital sepsis bundle improvement and morbidity/mortality reduction initiatives. Collects and reviews data to identify trends for benchmarking and performance improvement. Works collaboratively with hospital and medical staff to lead performance improvement activities in a manner that promotes a culture of learning and patient safety.
