A community based Senior Skills Trainer for our Personal Care AttendantProgram (PCA) in our Personal Care Management Department (PCM). TheSkills Trainer will educate consumers on how to successfully managetheir consumer-directed hands-on care. The Skills Trainer will also beprimarily responsible for other skills training department wide tasks &processes. Looking for Skills Trainer in the Plymouth County &Barnstable County areas in Massachusetts. Plymouth & Barnstable Countybilingual in Spanish highly preferred. Teaching background & knowledgeof the PCA program highly recommended. One year experience providingservices to people with disabilities highly preferred. Essential JobFunctions Follow-up on assigned referrals in a timely manner asdetermined by contract. Assess consumers\' ability to manage PCAservices. Train consumers &/or surrogates the skills needed to managetheir PCA program as outlined in the Mass Health Service Agreement &according to the Personal Care Management (PCM) Contract. Maintainconfidentiality in consumer related issues. Attend in-service,supervision & staff development meetings when requested by manager.Demonstrate a working knowledge of program policies & procedures & MassHealth regulations. Demonstrate a commitment to the Independent Livingphilosophy of consumer control. Must report all suspected incidents ofconsumer sexual/physical abuse & neglect to the Disabled PersonProtection Commission (DPPC). Work within company policies & inaccordance with the PCM Contract in order to complete & track forreceipt of required documentation within required timelines: Assignedreferrals & intake documentation Service Agreements & Assessments forre-evaluations Supportive documentation for intakes & re-evaluations asneeded Return all phone calls from surrogates & consumers within 48hours. Work within policies to complete & submit internal requirements:Progress Notes Outlook Schedule Mileage & Expense Forms Follow up withManager &/or Program Assistant on requests for skills training &tracking reports: Non-billing, overbilling & ineligible consumers Majorproblems requiring skills training Consumer status updates Quarterly &Annual Reviews Qualifications Experience providing services for personswith disabilities. Knowledge of PCA programs is preferred. Experience oreducation in teaching or counseling is helpful. Experience as a CNA,PCA, Case Manager, Coordinator, Medical background Knowledge ofcommunity resources & social service systems is beneficial. Excellentcommunication, organizational, & writing skills. Basic computer skillsin Microsoft Office/Outlook & the ability to navigate databases.Specialized training provided as needed. Required Education Bachelor\'sDegree preferred; GED or High School Diploma required.
