Schedule Details:Retail pharmacy experience preferred. Fulltime, 40 hours weekly. Works 10 hour shifts. Rotates every 3rd weekend.May require staffing at Mukwonago, Waukesha, Oconomowoc locations.Eligible for a \$3,000 sign on bonus! What You Will Do: Assistspharmacists, physicians and nurseswith medication reconciliationprocesses to ensure that any differences between medications takenbefore admission and medications ordered upon admission for a patientare intentional and void of negative patient incident. Assists withreconciliation process at discharge in coordination with pharmacist andcase manager.Offers discharge prescription service through ProHealthRetail Pharmacy. Collects appropriate insurance and demographic data forprescription processing.
