Schedule Details:Eligible for a \$3,000 Sign on Bonus! Works40 Hours per week, every 3rd weekend, 10 hour shifts. Earliest shiftstarts at 6am, latest shift starts at 10:30am. Shifts at WMH, MUK andOMH What You Will Do: Assists pharmacists, physicians and nurseswithmedication reconciliation processes to ensure that any differencesbetween medications taken before admission and medications ordered uponadmission for a patient are intentional and void of negative patientincident. Assists with reconciliation process at discharge incoordination with pharmacist and case manager.Offers dischargeprescription service through ProHealth Retail Pharmacy. Collectsappropriate insurance and demographic data for prescription processing.Responsibilities: Adheres to the general hospital standards to promote acooperative work environment by utilizing communication skills,interpersonal relationships and team building. Works cooperatively withothers and organizes daily workflow to optimize output. Completesmedication histories for new admissions. Promotes hospital\'s retailpharmacy program through offering of discharge prescription services.Provides service to customers to meet or exceed their needs andexpectations. Collects and compiles data, statistics and otherinformation needed for quality assurance programs or reports to provideinformation to assist with decision-making. Participates in the teamapproach to quality improvement to support the Pharmacy Department\'scommitment to the philosophy and principles of Total QualityImprovement. Maintains updated knowledge and skills and contributes tothe education of others.
