We are seeking a RN Oncology Navigator to: * Be a leaderin providing comprehensive cancer care for patients and caregiversthroughout all phases of cancer care. * Assess patients\' barriers tocare, psychosocial, and learning needs to advocate for and supportpatients and caregivers, collaboratively with teams and supportservices, as we strive to provide equitable care and outcomes for ourpatients. * Facilitate timely and coordinated transitions of care,provide anticipatory guidance, reinforce patient and caregivereducation, provide guidance in the shared decision-making process, andprovide mental and emotional support to patients and caregivers throughthe continuum of cancer care. * Participate in multidisciplinary tumorboard meetings. * Participate in continuous quality and processimprovement initiatives.
