Coordinates clinical activities of the Inpatient Wound Care Program which include but aren't limited to utilization of skills in organization, planning, implementing, evaluating, and providing care through use of hospital and nursing standards.
Essential Functions:
Performs assessment and plans patient care.Evaluates the patient assessment, planning and intervention process.Demonstrates professional and personal development.Delivers direct patient care appropriate to the age of patient served.Assess and diagnose wounds, ostomies and continence disorders.Develop and implement treatment plans.Monitor and evaluate patient progress.Provide patient education on self-care and disease management.Collaborate with other healthcare professionals to ensure holistic care.Stay up to date with the latest research and technology in wound, ostomy and continence care.Document patient progress and treatment plans accurately and thoroughlyProvide emotional support to patients and their families.Manage and maintain specialized equipment and supplies.Advocate for the needs and rights of patients with wounds, ostomy and continence disordersManage complex wounds, ostomies and continence issues.Minimum Education:
Graduate of a Program in Discipline - RequiredBachelor's Degree preferred.
Required Skills:Must be able to work independently.Addresses wound care consults timely.Ability to teach staff on wound care, staging, & documentationProfessionally interact with patientsAbility to teach patients how to manage their wound care.Facilitates Pressure Injury monthly meeting.Accurately assesses patient skin integrity.Prevent skin breakdown in hospitalized patients.Accurately stage and assess wounds.Microsoft office skills - Word, Excel, PowerPointCertifications:Basic Life Support (BLS) - to be obtained within 6 months.One to two years related experience and/or training preferred.
Required Licenses:Current licensure by WV State Board of Nursing.
Minimum Work Experience:Wound Care Certification preferred.Two years of experience in wound, ostomy and continence care required per Wound Ostomy Continence Nursing Certification Board.
